Grace Rehab & Wellness Centre (Grace Rehab) is a de-addiction and rehabilitation centre registered under the State Mental Health Authority, registration number C/257/2025/SMHA. We operate this website at gracerehabwellness.in.

What we collect

We collect information in only two ways through this website.

1. Information you give us in the contact form

When you submit the enquiry form, we receive:

your name;
your email address;
your phone number;
your preferred contact time, if you select one;
the message you write; and
a record that you ticked the consent box.

The message field is free text. Please share only what you are comfortable sharing — you do not need to disclose clinical details to start a conversation with us, and a phone call may be a better place for those.

2. Technical information

Like any website, our server records the IP address of requests. We use this only to limit how many times the enquiry form can be submitted from one address, which protects our helpline from automated spam. We do not build profiles from it.

This website does not use advertising cookies, analytics cookies, or tracking pixels. We do not set any cookie of our own.

How we use it

We use enquiry details for one purpose: to respond to your enquiry and, if you go on to seek treatment with us, to arrange that care. We do not sell personal information, and we do not share it for marketing.

Who else sees it

Two third parties are involved in the operation of this site. We have kept the list this short deliberately.

Web3Forms processes the enquiry form and delivers it to our email inbox. Your submission passes through their systems in transit.
Google Maps supplies the embedded map on our contact page. If you load that page, Google receives your IP address and may set its own cookies, under Google’s privacy policy rather than ours. No other page embeds Google content.

We may also disclose information where the law requires it, or where there is a genuine and immediate risk to someone’s life or safety.

Confidentiality of treatment

Separately from this website, the fact that a person is receiving or has received treatment with us is confidential. We do not confirm or disclose it to family members, employers, or anyone else outside the clinical team without written consent, except where the law compels us to.

How long we keep it

Enquiries that do not lead to treatment are retained only as long as needed to follow up, and are then deleted.

Your rights

You can ask us what information we hold about you, ask us to correct it if it is wrong, ask us to delete it, or withdraw your consent to being contacted. Withdrawing consent will not affect care already provided, and some clinical records may need to be retained even after a deletion request where the law requires it.

Children

This website is not directed at children, and we ask that the enquiry form be submitted by an adult. Where treatment involves a minor, we work with the parent or legal guardian.

Changes to this policy

If we change this policy we will update the date at the top of the page. Material changes will be highlighted here.
